Our Team

The patient is at the center of the care team’s focus. To assist in the recovery process, patients and their families should be involved in the care plan by attending scheduled meetings, providing information, asking questions, and participating in and complying with treatment protocols.

Members of Bridgeway’s interdisciplinary team include:

Physician – At Bridgeway, you will have an attending physician who will be overseeing your care. If necessary, your attending physician can call in one of the many specialists who are credentialed to consult at Bridgeway. Regardless of the reason you are at Bridgeway, you can feel confident in the team of medical professionals who are available to you. A physiatrist, a medical doctor specializing in physical medicine and rehabilitation, will communicate with the patient and their therapists to ensure the continuity of the rehabilitation plan. 

Physical and Occupational Therapists, Speech Language Pathologists – Physical therapists focus on strengthening, endurance, ambulation (walking) and coordination, while occupational therapists help increase the patient’s independence in bathing, grooming, dressing, eating, homemaking, and mobility. Speech language pathologists assist with speech, language and swallowing disorders as well as cognitive-linguistic and confusion and memory deficits.

Nurses – Nurses help patients manage medical problems, and they provide education to the patient and family. They also monitor the patient’s health and keep the patient and other team members updated on changes.

Social Workers – A social worker assesses each patient and starts the discharge planning process soon after admission, taking into account the patient’s lifestyle, routine and personal support system. This service can ease the transition back into the patient’s home and community. Social workers are also responsible for communicating information about the patient’s post-acute stay to their orthopedic or primary care physician, if the patient so chooses. In addition, they can help arrange for services and equipment after discharge.
